Important Patient Information

IMPORTANT INFORMATION PLEASE READ
Dear Patient

We are writing to you as a patient who is currently registered at Claypit Street Medical Practice to advise that as from 1st November 2017, Bridgewater Family Medical Practice is taking over the management arrangements for Claypit Street Medical Practice.

Due to ongoing difficulties with providing clinical staffing, Shropdoc took the difficult decision to terminate its contract with Shropshire CCG for patient services at Claypit Street in order to maintain safe services to patients. Shropdoc has been working closely with Bridgewater Family Medical Practice and the CCG to ensure a seamless handover.

Bridgewater Family Medical Practice is a friendly, forward thinking practice committed to improving the health and wellbeing of our patients of all ages. This letter is to welcome you and provides you with information on changes that will apply to you when accessing services from 1st November 2017.

Patients will be able to access services by contacting us Monday to Friday on either Practice number 01948 662097 / 01948 662870 and at either the Bridgewater site or the Claypit site. The opening times are detailed below:

Bridgewater Claypit
Monday 08:30- 18:00 Monday 08:30- 18:00
Tuesday 08:30 –18:00 Tuesday 08:30 -13:00
Wednesday 08:30 -13:00 Wednesday 08:30 -18:00
Thursday 08:30 – 18:00 Thursday 08:30 – 13:00
Friday 08:30 -18:00 Friday 08:30 -13:00

We have arranged to have access to your medical records from both sites so whichever site you contact you will have the information we need to meet your requirements. On the three afternoons that the Claypit Street site is closed, you can collect prescriptions and make appointments at Bridgewater.

To support the changeover patients are advised that it will be necessary for both practices to close at 12 noon on Tuesday 31st October 2017. This is to allow the computer systems that operate the appointments and medical notes to merge together. Urgent care cover will be in place after this time and patients should ring Shropdoc on 0333 222 66 55 in the first instance. The practices will both reopen on 1st November as normal.

You do not need to re-register to be a patient; however you will need to re-register for online services, prescription requesting, appointment booking and medical record viewing via patient access.

We apologise for this inconvenience and will provide extra administration staff to ensure request are dealt with promptly. From 2nd November you will be able to re-register either via the ‘ACCESS ONLINE SERVICES’ link at http://www.bridgewatersurgery.nhs.uk/ or directly from https://patient.emisaccess.co.uk/account/login and choose the register link. You will not require a letter from the practice as we will enable online registration; our post code is SY13 1QH which will need to be entered and Bridgewater chosen then follow the instructions to register.

We would also like to encourage patients to sign up for the electronic prescription service, which allows prescription requests to be transmitted direct to a pharmacy of your choice without the need for the paper copy being collected from the Practice. Please contact the practice or email us at bridgewater.admin@nhs.net entering the subject heading ‘EPS Nomination Request’ and providing your full name, address, date of birth and the name and town of the pharmacy you wish to nominate.

Should you have any queries on the content of this letter please contact either practice on the numbers provided above. If you have any queries regarding the change in the Provider of the contract, please contact 0800 032 0897 or SHRCCG.CustomerCare@nhs.net

Yours faithfully

Press Release: New management confirmed for Claypit Medical Practice in Whitchurch

Shropshire Clinical Commissioning Group (CCG) has announced that Bridgewater Family Medical Practice in Whitchurch will be taking over the management of the Claypit Street Medical Practice at Whitchurch Community Hospital, from Wednesday, 1 November 2017.

This announcement comes after Shropdoc, a not-for-profit company providing medical care, made the decision to hand the contract back to the CCG. Shropdoc had been providing primary medical services to the patients of Claypit Street Medical Practice since 1 November 2016.

Patients currently registered at Claypit Street will receive a letter informing them of the change in management including information on where and when GP appointments can be accessed at both GP practice sites.
Over the last few months, Bridgewater Family Medical Practice has been working in partnership with the Claypit Street Practice to provide additional support and to ensure that local patients continue to have access to GP appointments.

The next stage is the safe and smooth handover from the management of Shropdoc to the doctors at Bridgewater Family Medical Practice. Patients will remain registered at Clapypit Street Practice as it is the management arrangements that will change, not the registered practice.

The practices will reopen as normal on the 1st November 2017.

The CCG continues to support Whitchurch GPs with their vision to bring together all General Practice services in a single purpose built facility. Future proposals will be shared with local patients and the public in order to reach the best solution for Whitchurch patients.

Dr Julian Povey, Clinical Chair for the CCG, said:

“I would like to thank local practices for their rallying approach over the last few months, which means that patients at Claypit have continued to receive the care they need. I’m pleased that we have been able to secure new management at the practice and want to extend my thanks to both Shropdoc and Bridgewater Family Medical Practice for their cooperation and commitment to providing high quality primary care services to patients in Whitchurch.”

If patients have any queries they can contact Bridgewater Family Medical Practice on 01948 662097 or call the Patient Advice and Liaison Service (PALS) on 0800 032 0897.

Named Accountable GP

As from 1st April 2015 it is a contractual requirement that all patients on a GP Practice list are allocated a named GP.

At Bridgewater Surgery all patients have a named GP; this is the GP Partner that is recorded in your record as your usual GP.

This does not prevent you from seeing or speaking to any GP in the practice as you currently do now.

You do not need to take any further action, but if you have any questions, or wish to know who is recorded as your named/usual GP or to change your named/usual GP to one of the other GP Partners at Bridgewater please contact us by phone, email or post, as per the contact details at the top of this letter.

The GP Partners you can currently choose from are Dr James Muir or Dr Tim Lyttle.

Dr Mary Goudge is currently a salaried GP so is not able to be your named or usual GP
